PVSA Lab Privacy Policy
PVSA Lab values your privacy. This page explains what information we collect, how we use it, and the steps we take to protect your data. It also provides a dedicated description of our consent-driven notifications so reviewers can verify our toll-free messaging use case.
Information We Collect
- Account details: Name, email, phone number, provider association, and practice information submitted during registration.
- Clinical workflow data: Test orders, collection kit status, account activity, and communication preferences managed in the PVSA portal.
- Device and security data: Log records and analytics used to secure the platform and troubleshoot issues.
How We Use Information
- Deliver PVSA testing services and maintain your account.
- Send clinically relevant updates, including order confirmations, collection instructions, result availability, and account status notices.
- Meet legal, regulatory, and security obligations.
Consent for Notifications
We only send account status notifications to phone numbers that have explicitly opted in. Consent is captured through our authenticated portal at sign-up or when a user updates their communication settings. Users must acknowledge a clear disclosure stating that PVSA Lab will send account status and billing-related notices from our dedicated toll-free number and that standard messaging rates may apply.
How Consent Is Captured
- Explicit opt-in: Users select a checkbox authorizing SMS/voice notifications for account and billing status updates before we enable messaging to their number.
- Traceable audit log: Each opt-in event records the user ID, phone number, timestamp, IP address, and acceptance text so verifiers can confirm consent history tied to the toll-free number.
- Accessible disclosure: The consent language is available in the communication settings screen and on this privacy page for review.
Example Consent Form (Read Only)
View-only for verificationThe following is a snapshot of the in-app form patients complete to authorize account status notifications from our toll-free number. The controls below are non-interactive to preserve the exact disclosure and layout used in production.
Our Toll-Free Notification Use Case
PVSA Lab uses a single toll-free number to deliver patient account status notifications (for example, portal activation, collection kit shipment updates, payment issues, or result availability reminders). The number is not used for marketing. All messages are strictly transactional, linked to the patient's PVSA Lab account, and only sent after the opt-in described above. Verification teams can review this page to confirm the scope and consent model of our toll-free traffic.
How to Withdraw Consent
- Reply STOP to any SMS to halt future notifications.
- Update communication preferences in the PVSA Lab portal or contact support to disable messaging for a specific number.
- Opt-outs are logged immediately and enforced across all future toll-free messages.
Data Sharing & Protection
- We do not sell personal data. Information is shared only with service providers supporting PVSA Lab operations under contractual safeguards.
- We apply industry-standard security controls, including encryption in transit and at rest, role-based access controls, and continuous monitoring.
- Users can request copies of their data or deletion where permitted by applicable law.
